Renewal of Passport/Damaged Passport (Adults):

• Completed passport application form
• Two (2) identical color photographs one of which should be certified by the Certifying Official who signs Section F
• Expired Passport
• Certified copy of Birth Certificate
• Certified copy of Marriage Certificate, where applicable
• Certified copy of Death Certificate, where applicable
• Certified copy of Adoption Register, where applicable
• Divorce Judgment, where applicable

If born abroad, ,instead of the Birth Certificate, please submit one of the following:

• Certificate of Naturalization
• Letter of Certificate of Citizenship
• Certificate of Registration

• Jamaican driver's license

• Cancelled Jamaican Passport

• Jamaican voter's registration card

• Work Permit bearing a photograph from the country of residence

• Passport from country of residence

• Citizenship Certificate from country of residence

• Identification indicating permanent residency

• Driver's License from country of residence

• Other identification issued by a Federal/State/City authority which has an
  expiration date (subject to approval by a Consular Officer)

Lost/Stolen Passport (Minors):

• Completed passport application form

• Two (2) identical color photographs one of which should be certified by the Certifying Official who signs Section F

• Certified copy of Birth Certificate, ,of Letter of Certificate of Citizenship, if born abroad

• Certified copy of Adoption Register, where applicable

First-Time Applicant (Adults):

All first time applicants must visit the Consulate General to be interviewed by a Consular Officer. 

• Completed passport application form
• Two (2) identical color photographs one of which should be certified by the Certifying Official who signs Section F

• Certified copy of Marriage Certificate, where applicable
• Certified copy of Adoption Register, , where applicable
• Divorce Judgment, if applicable

Amendment of Jamaican Passport:

Addition of Married Name (Women Only)

• Completed application form
• Current Jamaican Passport
• Certified copy of marriage certificate
• Original divorce decree or death certificate (Where applicable)

Extension of Validity

• Current Jamaican Passport
• Completed extension of validity application form

Change of Name

• Current Jamaican Passport
• Deed Poll

Mailed Applications:

Section F and one (1) photograph must be certified by a Notary Public. Their stamp should be affixed in the area provided. These persons need not be Jamaicans or Commonwealth Citizens.

NB: Photocopies of documents, even if notarized, will not be accepted. All original documents will be kept and returned at the end of Passport processing period.

Passport Fees:

The Consulate General of Jamaica accepts only Money Orders and Cash (Do not send cash in the mail). Personal checks and credit cards are not accepted. The processing time for Passports is six (6) weeks.
